1. What if we did not comply or past the surveillance audit what are the consequences that we have to face.

2. Can we still continue for re-certification and upgrade to 9001:2015 version (9001:2008 are current certificate to be expired on April 2018) and 13485:2016 version (13485:2012 are current certificate to be expired on April 2018) is it okay to upgrade even we did not past the 4th Surveillance Audit.

Answer:

1. if you don't pass the surveillance audit, you wont be able to maintain your certificate, thus your certificate will be revoked.

2. Yes, you can continue towards the transition although you didn't passed the surveillance audit. In any case, certification against new version of the standard is considered as a new certification and surveillance audit against old versions of the standard is not related to it in any way.

Thank you for that great details.
I have another question. Since we have plan to upgrade are standard in to new version by next year is it possible to cancel are upcoming surveillance audit from old version and apply for re-certification audit for new version. Are surveillance audit is September 2017 for 9001:2008 and 13485:2012 and the expiration of this is on April 2018. before April 2018 we have to apply for re-certification audit under 9001:2015 and 13485:2016. Is it okay if we disregard the september 2017 surveillance and move to re-certification before the expiration of are certificate.

Yes, you can cancel the surveillance audit and apply directly for the new version. The only thing is that between the moment when you cancel the certification audit and get certificate for the new version, your company wont be ISO 9001 certified.
