Coding the documents during transition to ISO 9001:2015
Can I get some further clarification on this comment. We have an existing QMS system so we should put the existing document code on the "Code" line even if the existing code will be different going to the new standard?

Answer:
The new version of the standard does not prescribe the coding method, so the organization can use the same coding. The only thing that needs additional caution are the existing documents that need to be revised, those document can keep the same coding but the version number must be changed and the organization must ensure that the new versions are available on the place of application and the old version are withdrawn.

Thanks for the reply, so for example with Management Review in 2008 we have it documented according to the standard as 5.6, but with moving to 2015 standard wouldn't we want to re-code Management Review to 9.3 so it matches the standard?

Yes, exactly and also you need to add new management review inputs, such as :
- changes in external and internal issues that are relevant to the quality management system;
- the effectiveness of actions taken to address risks and opportunities
