GDPR serving citizens abroad

I am employed by a company with offices n Canada and UK... I am a lone employee of my company and their plan is to have me support their clients - who are nurses who use our software in documentingon their residents. What's the best thing to do to make this journey serving UK clients who serve residents/patients in different healthcare settings short and sweet, and more importanly, possible.

Your company would need to sign a Data Processing Agreement with the entities using the software.

Because you are in Canada and Canada received adequate status from the EU Commission there is no issue with the data transfer.
