Effectiveness of actions

My company, is a client and we used your templates to create all of our ISO 9001-2015 documents. During the QMS Review we are supposed to report on the "Effectiveness of actions taken to address risks and opportunities". Do you have example of how that might look or what would be reported? The word "effectiveness" means to me that we need to analyze something that may or may not be in a spreadsheet. Any insight to an easy way to do this would be very much appreciated.

You can monitor and measure the actions analysing and evaluating the data gathered in order to determine their effectiveness. Your organization can use different methods to do that, such as Key Performance indicators, business metrics, internal audits, monitoring of corrective actions and action plans and subsequent reporting into management reviews.
