Why do ISO9001 standard only require "Effective", not "Efficiency"?

I will answer with my own opinion.
ISO 9001 started as a quality assurance standard. With ISO 9001 implemented, potential customers receive a sign that the supplier is able to deliver what it is promised, that it is effective. Efficiency, from customer’s point of view, is translated into a competitive price, but price is transparent since the beginning.
