Which template should be used as a standard one for the Process Owner to submit for Internal Review Process (i.e i mean internally by ITSM). Actually, I'm doing for the Recertification of ISO/IEC 20000: 2011 -1 so the template should have the past 3 years of KPI, trending on maturity & effectiveness of ITSM process.

Answer:
If I understood correctly, you'd like to set-up the process for Internal review. If you put all KPI's (for all ITSM processes) in one document - that will be very large document. I would suggest to include KPI's in every single process document.

What i is mean is a standard template for all ITSM processes to show/present their KPI as as evidence of efficiency and maturity.
I mean in the template what are items to put in.
